During this session, we will:
- Review all areas of need using our ElderCARE Clarity Model, including health & medical, care coordination, home & environment, daily functioning, financial and insurance considerations, legal decision-making, and social support
- Identify immediate risks, barriers, and key priorities
- Develop a personalized, written plan with clear, actionable next steps
Guided Care Assessment: $300
This assessment serves as the foundation for all ongoing care coordination and advocacy services.

Care Navigation & Advocacy
Ongoing coordination and alignment of care, services, and resources—ensuring families can move forward with clarity and support.
This service includes:
- Up to 6 hours of care coordination and advocacy
- Communication with healthcare providers, community partners, and insurance contacts
- Scheduling support and follow-through on recommended services
- Serving as your family’s professional point of contact
- Clear, timely updates following key interactions
Ideal for: Families navigating new diagnoses, coordinating multiple services, or experiencing uncertainty around next steps
Coordinated Transitions
Enhanced support for complex transitions requiring thoughtful planning, communication, and continuity of care.
This service includes:
- Up to 10 hours of intensive transition support
- Review and implementation support for discharge plans and clinical recommendations
- Communication across all care partners, including hospital, rehabilitation, specialists, and home care providers
- Guidance in evaluating long-term care options, including home care, personal care, memory care, and skilled nursing
Ideal for: Hospital-to-home transitions, rehabilitation discharges, changes in cognition or mobility, or situations where safety and stability are at risk

Care Continuity Retainer
Ongoing support designed to ensure consistency, oversight, and connection in your loved one’s care—especially when families are not local.
This service provides steady, professional guidance following an initial care plan or transition, helping families stay informed, supported, and confident over time.
This service includes:
- Up to 4 hours of ongoing, scheduled support each month
- Continued coordination, communication, and oversight of care needs
- A consistent professional point of contact for your family
Comprehensive Continuity Retainer
Expanded, high-touch support for families who require more frequent communication, coordination, and oversight.
Designed for more complex or evolving situations, this level of support ensures continuity, responsiveness, and deeper involvement in your loved one’s care.
This service includes:
- Up to 8 hours of ongoing, priority scheduled support each month
- Increased communication and coordination across care providers and services
- Enhanced availability and responsiveness for changing needs
Concierge Hourly Support
$125 per hour*
Offered on a limited basis for clearly defined, short-term needs when appropriate. Standard rate for hourly CAREgiver Support Sessions.
*$25 additional per hour on weekends

The Family CARE Conference is a collaborative, guided planning session that brings loved ones together with Compassion, Advocacy, Resources, and Empowerment at the heart of the experience. In this facilitated conversation, we help families gain clarity around current needs, anticipate future challenges, and understand the options available across medical, legal, financial, and community domains.
With a focus on strengthening communication and reducing overwhelm, we align family members around supportive roles, accessible resources, and a shared vision for aging well. The result is a confident, intentional actionable plan that emphasizes preparedness, transforms uncertainty into empowerment—ensuring care decisions are made with compassion, not during crisis.
Medicare: Navigating Insurance Benefits & Plans
ElderCARE Collaborative offers education-first, trauma-informed Medicare coverage reviews to help individuals understand how their benefits align with real-world care needs. Complimentary reviews include clear explanations of current coverage and plan comparisons to support informed, pressure-free decision-making. There is no cost for enrollment assistance or ongoing service for Medicare Supplement, Medicare Advantage, and Medicare Part D Prescription Drug Plans.
As a licensed Medicare insurance agent, I may provide education and enrollment assistance for Medicare plans. We do not offer every plan available in your area. CAREgiver Support & Advocacy
Caregiving can be complex and emotionally demanding—especially for adult children and family members supporting an aging loved one. This service offers non-clinical, concierge-level support through empathetic listening, practical guidance, and steady advocacy, creating space for the individual caregiver to reflect on their role, responsibilities, and evolving expectations. The focus is on helping caregivers gain clarity, perspective, and balance as they adjust to a significant life transition.
