ElderCARE Collaborative is a private-pay, concierge aging advocacy service that supports senior adults and their families through planning, transitions, and complex care decisions. We provide guidance, organization, and advocacy—helping families navigate aging with clarity and confidence.
How are your services different from a care manager or home care agency?
ElderCARE Collaborative focuses on non-clinical advocacy and implementation support. We help families understand recommendations, coordinate trusted resources, and follow through on plans. We do not provide direct homecare staffing or clinical case management, and we are independent of home care agencies.

Do you provide medical or clinical care?
No. ElderCARE Collaborative is a non-clinical service. We do not provide medical treatment, diagnosis, or therapy. Our role is to support families through education, coordination, and advocacy—working alongside medical, legal, and care professionals, not replacing them.
How do you work with Medicare or insurance coverage?
Our services are private pay and not covered by Medicare; therefore no need to obtain insurance authorization. You have complete control over your level of engagement.
However, we provide education and guidance related to Medicare and aging-related coverage options. When applicable, as a licensed insurance agent, we provide Medicare plan review, enrollment and ongoing service which are conducted in compliance with all regulatory requirements. There is no service charge for assistance with enrolling or servicing your Medicare, Medicare Supplemental, Medicare Advantage or Part D Prescription Drug plan.
I do not offer every plan available in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options.
How do you work with other professionals involved in my loved one’s care?
We collaborate respectfully with physicians, discharge planners, elder-law attorneys, financial professionals, real estate agents and care providers. Our role is to help families implement and understand recommendations, ensuring continuity and reducing confusion during transitions.
Is your service confidential?
Yes. ElderCARE Collaborative is committed to privacy, discretion, and ethical handling of client information. While we are non-clinical, we follow best practices for confidentiality, obtain releases and secure documentation appropriate for the services we provide.
How do I get started?
We begin with an initial complimentary intention to understand your situation, priorities, and support needs. From there, services are tailored to align with your goals, timeline, and level of support desired.
