- Scheduling support for services or follow-up needs
- Serving as the family’s professional liaison
- Detailed summary updates after each major interaction
Ideal for: Families managing new diagnoses, resource coordination, or safety uncertainty
Adapt: Coordinated Transitions
High-touch oversight for major life transitions requiring thoughtful logistics and continuity
- Includes up to 10 hours of intensive transition support
- Review and support in implementing discharge plans or clinical recommendations
- Communication with all care partners: hospital, rehab, specialists, home care, etc.
- Guidance evaluating long-term care options (home care, personal care, memory care, skilled nursing, etc.
Ideal for: Hospital-to-home transitions, rehab discharges, shifting cognition or mobility, or when safety/stability are at risk.

Continuity Retainer
Ongoing support designed for families who are geographically separated, but want consistent oversight and connection in their loved one’s care plan. This is ideal for when families have to return home after an acute event, but appreciate ongoing, local professional oversight and support.

Concierge Hourly Support
$125 per hour*
Offered on a limited basis for clearly defined, short-term needs when appropriate. Standard rate for hourly CAREgiver Support Sessions.

The Family CARE Conference is a collaborative, guided planning session that brings loved ones together with Compassion, Advocacy, Resources, and Empowerment at the heart of the experience. In this facilitated conversation, we help families gain clarity around current needs, anticipate future challenges, and understand the options available across medical, legal, financial, and community domains.
With a focus on strengthening communication and reducing overwhelm, we align family members around supportive roles, accessible resources, and a shared vision for aging well. The result is a confident, intentional actionable plan that emphasizes preparedness, transforms uncertainty into empowerment—ensuring care decisions are made with compassion, not during crisis.
Medicare: Navigating Insurance Benefits & Plans
ElderCARE Collaborative offers education-first, trauma-informed Medicare coverage reviews to help individuals understand how their benefits align with real-world care needs. Complimentary reviews include clear explanations of current coverage and plan comparisons to support informed, pressure-free decision-making. There is no cost for enrollment assistance or ongoing service for Medicare Supplement, Medicare Advantage, and Medicare Part D Prescription Drug Plans.

CAREgiver Support & Advocacy
Caregiving can be complex and emotionally demanding—especially for adult children and family members supporting an aging loved one. This service offers non-clinical, concierge-level support through empathetic listening, practical guidance, and steady advocacy, creating space for the individual caregiver to reflect on their role, responsibilities, and evolving expectations. The focus is on helping caregivers gain clarity, perspective, and balance as they adjust to a significant life transition.
